I think it would be more realistic if we added half a heart or even a heart (or two) of damage when someone smashes a splash potion at their feet, even a health or regen potion.

The reason why is because smashing glass bottles at your feet is not the safest thing in the world. For the sake of making this more realistic. Maybe instead of having the damage happen every time it could happen once every 10 or 20 times?

I do see why this would not be needed and how pointless it really is, but maybe this would even encourage pvpers to use something other than health potions to heal them. It could create a sudden rp opportunity (although minimal at best) at a character's minor injuries that was not expected.

Thoughts about this very minor idea?

Hmm, I could see this (realism wise) to be a thing, however most of PvP'ing happens in diamond armour (which for some reason is more durable than iron in Minecraft.) You'd have to make it only a barefeet or at best also leather boots only thing, at which it would become a rather unused mechanic as far as I can see.
For the difficulty of making such a mechanic, I wouldn't know, not a Tech Staff
For the usefullness in PvP, neither Game Staff nor PvPer

Really, I don't think its realistic. There's no need to encourage PvPers to use something other than health potions, especially since health potions are already expensive to make. On the realistic side, PvPers have boots made out of diamond. If the diamond boots don't protect them, they would at least shred their feet to pieces without there even being any glass. Roleplayers can feel free to roleplay out stepping on glass if they want I geuss

From the tech side of things this would be a 15 -min change to implement, however speaking from my knowledge of pvp, realism isn't exactly a great concern, and wouldn't really be a good idea to put in.

Maybe for the sake of realism I don't think it's a good idea. But to encourage the use of drinkable potions (slower) there is a small chance the splash effect can backfire and either not heal as much or not heal at all. The risk if you want to use splash potions is up to you or maybe even both. Further testing and conversation is needed however.

Lag on Massive makes it impossible to either drink or eat at times so no thank you
For example:

I don't even do survival actively, but the thought of getting damage from doing something normal in minecraft is not enjoyable.

For the sake of arguing the merit based on the OP's stance, players wear armor, so any stepping on glass would be ineffective already. One could say adding damage to pots is less realistic, but I digress.
